1. What ClaimFighter Is

ClaimFighter is an AI-powered tool that helps you draft insurance appeal letters. We generate appeal letters based on your denial documents, historical case data, and applicable regulations.

Important: ClaimFighter is not a law firm, medical provider, or insurance company. Our appeal letters are AI-generated drafts. We do not provide legal advice, medical advice, or guarantee any outcome. You should review every letter before sending it and consult a professional if needed.

2. Your Account

  • You must provide a valid email address to create an account.
  • You are responsible for maintaining the security of your account.
  • You must be at least 18 years old to use ClaimFighter.
  • One account per person. Do not share your account.

3. Credits and Billing

  • You receive 2 free credits upon signup.
  • Each case submission costs 1 credit.
  • Subscription plans grant monthly credits: Basic (3/mo), Pro (10/mo), Team (25/mo).
  • Credits expire 12 months after being earned or granted.
  • If processing fails, your credit is automatically refunded.
  • Subscriptions are billed monthly through PayPal. You can cancel anytime.
  • Earned credits (from uploads, referrals, outcomes) are capped at 5 per month to prevent abuse.

4. Your Documents

  • You retain ownership of all documents you upload.
  • You grant us a limited license to process your documents for the purpose of generating appeal letters.
  • You are responsible for ensuring you have the right to upload any documents you submit.
  • We store your documents securely and delete them upon your request.

5. AI-Generated Content

  • Appeal letters are generated by AI and may contain errors, outdated information, or inaccuracies.
  • You are solely responsible for reviewing, editing, and sending any appeal letter.
  • We do not guarantee the accuracy, completeness, or effectiveness of any generated content.
  • Success probability estimates are based on historical data and are not predictions of your specific outcome.

6. Knowledge Base Contributions

When you mark an appeal outcome (won or lost), we use anonymized data from your case to improve our AI. This means:

  • Your personal information (name, address) is never included.
  • Only denial codes, insurer names, and appeal strategies are retained.
  • This data helps future users with similar denials.
  • You earn credits for contributing outcomes.

7. Prohibited Use

  • Do not upload fraudulent or fabricated documents.
  • Do not use ClaimFighter to file appeals you know to be without merit.
  • Do not attempt to manipulate the credit system or abuse the referral program.
  • Do not use automated tools to submit cases in bulk.

8. Limitation of Liability

ClaimFighter is provided "as is" without warranties of any kind. We are not liable for:

  • The outcome of any insurance appeal.
  • Errors or inaccuracies in AI-generated content.
  • Missed deadlines if you rely solely on our deadline tracking.
  • Any damages arising from use of the service.

Our maximum liability is limited to the amount you paid us in the 12 months preceding the claim.

9. Termination

You can close your account at any time. We may terminate accounts that violate these terms. Upon termination, your data will be deleted within 30 days.
